A botched BBL is a Brazilian butt lift whose result is still wrong once healing is complete: one side larger or higher than the other, hard lumps, a shelf where the lower back meets the buttock, dents where fat was taken, or projection that is too much or that mostly disappeared. Most of these can be improved, though rarely erased, with revision liposuction and fat grafting. A few are medical emergencies rather than cosmetic problems, and this page separates the two.
What a Botched BBL Looks Like
Asymmetry
One side is fuller, higher or shaped differently from the other. Some unevenness is normal early, because swelling resolves unevenly and most people sleep on one side, but a difference still visible at six months lives in the fat that was placed or survived.
Lumps, Hard Spots and Fat Necrosis
Transferred fat lives only where blood vessels can reach it, at the outer edge of each small deposit. Fat placed in large pools dies in the center and turns into firm lumps, oil cysts or calcified nodules, a process called fat necrosis. Small firm areas soften over months; a lump that grows, hurts or drains needs to be seen.
Over-Projection and the Shelf
Too much volume, especially placed high and centrally, gives a step at the top of the buttock rather than a curve. The same shelf appears when liposuction of the lower back stopped short and left a ridge of fat where the waist should flow into the buttock.
Dents, Waves and Cannula Marks
The harvest areas suffer most from aggressive or uneven liposuction: grooves and dents in the flanks and back, a wavy surface, indentations stuck to the muscle beneath, and scarred marks where the cannula passed too close to the skin. Many patients are more troubled by the donor areas than by the buttock.
Fat That Mostly Disappeared
Some loss is expected in the first three months as fat without a blood supply is reabsorbed. Losing most of it points to fat damaged during harvest, an over-filled pocket, pressure from sitting or tight garments early, or weight loss afterward. Our post on why a BBL shrinks goes through each cause.
Normal Healing Against a Real Problem
Judging a BBL too early is the most common mistake. In the first two weeks the buttocks are swollen, firm and larger than the final result. Between six and eight weeks the swelling leaves and the fat softens, which patients call "fluffing". By three months the fat that will survive has survived, and the shape refines for up to a year. Numbness, small firm patches and slight unevenness in that window are healing; our BBL recovery guide sets out what is normal week by week. A shelf, a dent, a hard lump or a clear asymmetry still there once the swelling has gone is not healing, and neither is any odor: a healing BBL does not smell, and a foul odor from an incision or drainage means infection or liquefied dead fat.
Why It Happens
Fat placed in the wrong plane is the first cause and the dangerous one. Injected into or beneath the gluteal muscle, fat can enter the large veins there and travel to the lungs, which is how BBL deaths happen; the plastic surgery societies' task force on gluteal fat grafting set the rule that fat goes only into the fat layer above the muscle. Fat placed too shallow or in big boluses gives lumps. Too much volume is the second: a buttock can nourish only so much grafted fat, and the rest dies, so the over-filled result loses the most. A poor harvest is the third: aggressive suction damages the donor area and the fat itself, and uneven liposuction leaves the shelves and steps above.
Behind all three sits an inexperienced or hurried operator. High-volume clinics that book several BBLs a day, what Dr. Cat calls "lipo factories", and clinics in Mexico, Turkey, Colombia and the Dominican Republic that advertise a fraction of the price account for most revisions we see. The American Society of Plastic Surgeons and the CDC have both warned about medical tourism complications, and a revision of overseas surgery costs more than the original operation would have. What Is Dangerous and What Is Cosmetic
Most bad results are cosmetic and not urgent. Four things are. Sudden shortness of breath, chest pain, a racing heart or confusion in the first days after surgery can mean fat has reached the lungs: call 911. Spreading redness, warmth, fever, worsening pain after day three or foul-smelling drainage means infection and needs a doctor the same day. A hard area that grows and drains oily fluid is fat necrosis that may need draining. Calf pain or swelling in one leg can be a clot. None of these wait for a reply from a clinic in another country. Asymmetry, a shelf, dents and lost volume are the opposite: they wait, and waiting improves the revision.
When to Wait and When to Act
For cosmetic problems the rule is six to twelve months from the original surgery, long enough for the swelling to leave and the surviving fat to declare itself. Use the time: photograph yourself monthly from the front, side and back in the same light, request the operative report from the first surgeon, and stay away from massage devices and "fluffing" gadgets, which can make irregularities in scarred tissue worse. How a Botched BBL Is Fixed
The operation is revision liposuction in Beverly Hills with fat grafting, a surgery Dr. Cat specializes in. Under general anesthesia she uses a custom-crafted instrument to break up the scar tissue from the first surgery and harvest fat, through incisions under 4 mm, reusing the old ones where she can. Shelves and lumps are removed with a thin cannula and blended into the waist, flanks and back; dents are lifted and filled with fat grafts; asymmetry is corrected by adding to one side, reducing the other, or both. A large correction is sometimes staged. Patients sometimes ask whether an implant or Sculptra could rebuild a buttock that deflated or was over-suctioned. Dr. Cat does not place buttock implants, does not inject Sculptra or any filler into the buttocks, and does not perform buttock lifts; a correction is done with your own fat, because it is the only material that becomes part of the tissue it is placed in and can be blended into scarred ground without adding a device that can shift, harden or be felt through a thin frame. If there is not enough fat to donate for the correction you want, she says so at the consultation. Recovery is lighter than most patients expect: walking within hours and work in a few days for many. "Fixed" means improved: over-removed tissue and skin damaged by high suction cannot always be fully restored, and Dr. Cat says so at the consultation rather than after. How do you know if your BBL is botched?
Swelling, firmness, some early volume loss and mild unevenness are normal in the first three months. A shelf, dents, hard lumps, a clear size difference or a result that mostly disappeared, still there at six months, is a problem.
Can a botched BBL actually be fixed?
Usually it can be improved, which is not the same as erased. Shelves, lumps, dents and asymmetry respond to revision liposuction and fat grafting; over-removed tissue and skin damaged by aggressive suction cannot always be fully restored.
How long do I have to wait to fix it?
