What is a skinny BBL?
Every BBL has three parts: liposuction to harvest the fat, washing so that only clean, living tissue goes back in, and the transfer into the buttocks and hips in small passes. A skinny BBL keeps all three; what changes is arithmetic. A patient with generous donor areas can give up liters of fat, while a patient with a BMI of 20 yields a fraction of that, so more areas are harvested for less fat each, nothing is over-suctioned, and the transfer is placed for shape rather than spread across the whole buttock for size. Our guide to what a BBL is covers the operation. If you have plenty of fat to donate and want a fuller result, the Brazilian butt lift page describes that operation; this page is for the patient who has been told she is too thin for one.
What a skinny BBL is not
It is not an implant operation: Dr. Cat does not place buttock implants or inject Sculptra or any filler into the buttocks. Fat is her only approach, because it is your own tissue and it slims the donor areas at the same time; if there is not enough to make a visible difference, she says so at the consultation, and she has turned patients away for that reason. It is not weight loss, and it is not a lift: loose skin needs a buttock lift, which Dr. Cat does not perform; our page on flat or saggy buttocks explains the difference. And it is not a smaller surgery: the hours, the six-week rules and the price range are those of a full BBL, because the work is in the harvest.
Who is a good candidate for a skinny BBL?
The first requirement is donor fat, and the skinny BBL exists because "enough" is a smaller number than it used to be. The American Society of Plastic Surgeons has described the shift: surgeons who once turned away anyone with a BMI under 22 or 23 now plan for shape rather than volume, and the patients asking are often fit, or have lost weight, increasingly on GLP-1 medications. In our experience patients with a BMI between about 18 and 23 can see a meaningful change. Our comparison of liposuction vs BBL covers that decision.
You may be a good candidate if
- A BMI in the high teens to the low twenties, and buttocks that are flat or square, often with hip dips
- Pinchable fat at the waist, lower back, flanks or inner thighs, even in small amounts
- Shape and proportion as the goal, not a dramatic increase in size
- A stable weight close to your goal
- No nicotine, and six weeks without sitting directly on the buttocks
Not yet, or a different plan, if
- Not enough fat to make a visible difference; Dr. Cat says so at the consultation
- Loose skin rather than missing volume: that is a buttock lift, which Dr. Cat does not perform
- A large increase in projection: that plan needs the fat to give, and it is a full BBL
- Smoking or vaping: Dr. Cat requires two months to a year without nicotine before surgery
- The same day as a tummy tuck or a breast augmentation: those combinations are staged on separate days
Should you gain weight before a skinny BBL?
Patients ask whether to gain ten or fifteen pounds first so there is more to harvest; the guidance runs the other way. Weight gained on purpose goes where your genetics send it, not only to the donor areas, and when you lose it afterward every fat cell shrinks, including the ones moved to your buttocks, so a result built on borrowed weight deflates with the diet that follows. Dr. Cat asks patients to be close to their goal weight before surgery and stable afterward; the medications to stop two weeks before are in our pre-op instructions.
How Dr. Cat does it
Dr. Cat is a board-certified plastic surgeon who completed two surgical residencies, one in head and neck surgery, microvascular and facial reconstructive surgery, and advanced training in general plastic and reconstructive surgery, Harvard-educated and UCLA trained. She performs every step herself, and because she also specializes in revision liposuction, she sees every week what over-aggressive fat removal leaves: dents and injured skin that are often irreversible.
The harvest: a liposuction 360 on a lean frame
Her most common BBL plan is a liposuction 360 of the back, waist and abdomen, and for a slim patient that harvest is the whole point. The waist becomes the narrowest point and every liposuctioned area blends into the next; a transfer that would look small on a fuller frame reads as a real curve beside a narrowed waist. The fat is removed with thin cannulas using tumescent and power-assisted liposuction, never a heat device, with her multi-phase technique: gently broken into tiny particles, removed evenly on a precise grid with every stroke counted, then blended a final time. Each area gives a little, and she stops well short of the over-suctioning that leaves dents.
The transfer: shape before size
The fat is gently washed rather than spun, so only intact fat goes back in, and placed in many small passes, in the layer under the skin and above the gluteal muscle, never into it, with a blunt cannula. On a slim patient the volume goes where it changes the silhouette most: the upper buttock, the outer curve and the hips. Hip dips are common on lean frames, and she grafts the hips in the same operation when the plan calls for it. She asks which shape you want, a rounder profile or an athletic heart-shaped contour, turns you during surgery to check every angle, and says so when a goal photo has been edited. In Dr. Cat's experience more than 80 to 90 percent of the transferred fat survives, which is why her patients rarely need a second round.
Surgery day, one patient per day
Surgery is under general anesthesia with a board-certified physician anesthesiologist in Dr. Cat's QUAD A accredited surgery center on Rodeo Drive, with a numbing solution in every operative area. The incisions are under 4 mm, closed with dissolvable sutures. Plan on about four to seven hours depending on the number of areas: the time is in the liposuction, and a lean frame takes more areas, not fewer. She schedules one liposuction with fat transfer per surgical day and nothing else, and a BBL is never performed on the same day as a tummy tuck or a breast augmentation; those combinations are staged on separate days. You wake in a compression garment with foam over the liposuctioned areas and spend the first night at a plastic surgery aftercare center, whose nurses bring you to the office the next morning.
Mini BBL: the same operation with a different name
"Mini BBL" is a marketing term, and there is no smaller surgery to shop for. Some clinics use it for a smaller transfer in any patient who wants subtlety, and skinny BBL for a patient with little fat to give; others use the two interchangeably. Either way the operation is liposuction with fat transfer under general anesthesia, with the same incisions, fat survival, six weeks without sitting and range of cost, because the work is in the harvest. Nor is it a non-surgical BBL: buttock filler is a different product with a temporary result, and Dr. Cat does not offer it.
Skinny BBL recovery timeline
Recovery after a skinny BBL is a BBL recovery: the volume is smaller, the rules are not, and the liposuction covers more of a lean frame. Our BBL recovery guide has the week-by-week version.
- The night of surgeryYou wake in your compression garment, numb rather than sore, and go to the aftercare center, where nurses get you up to walk and keep you off your back.
- Days one to threeThe aftercare center brings you to our office for your first visit. Expect soreness across the liposuctioned back, waist and abdomen and tight, heavy buttocks; most patients manage with Tylenol. You sleep on your stomach or side and sit only on the BBL pillow.
- One weekBruising and swelling peak. Most patients are off all pain medication and back at desk work on the pillow, standing every half hour; our post on how to sleep after a BBL shows the positions.
- Two weeksDr. Cat checks the incisions and starts scar treatment with products the office provides. Driving resumes at two to three weeks on the pillow; out-of-town patients usually fly home around now, once cleared.
- Weeks three to sixMost normal activities are back by week four. The garment stays on and the pillow goes everywhere, because pressure on fat that has not yet grown a blood supply reduces how much survives. No squats, cycling or floor abdominal work, and no dieting.
- Six weeks to three monthsAt six weeks the rules end together: you may sit directly on the buttocks, lie on your back and take the garment off, and exercise resumes with Dr. Cat's clearance. Between six and eight weeks the shape rounds and softens, the stage patients call fluffing; take your first real photos at three months.
- Six months to one yearThe last swelling leaves the waist, the scars fade toward skin color, and the shape you see is the shape you keep, at a stable weight.
Skinny BBL results, and how long they last
The result of a skinny BBL is a line rather than a size: a narrower waist, hips that run smooth into the thigh, and a buttock that is rounder at the top and the outer curve, in proportion to a small frame. The fat that survives is your own permanent tissue, and the final shape shows at six to twelve months. Weight changes move it the way they move the rest of you: the donor areas have fewer fat cells for good, so a later gain lands proportionately less at the waist, while the transferred fat enlarges with a gain and shrinks with a loss. Dr. Cat tells patients a BBL can last ten years and more without a significant gain or loss; our post on what happens if you gain weight after a BBL explains why.
Skinny BBL before and after photos
The pair below is one of Dr. Cat's patients, a 24-year-old with a lean build kept that way by exercise and diet. Fat was harvested from her waist and thighs, and the volume moved was small. In the after photo her buttocks are not much larger; they are rounder and lifted, the thighs more defined, with a slight inward curve where the thigh meets the buttock. The change is shape.

More cases are in our BBL before and after photos; look for the slimmer frames and compare the waist as well as the buttock, because the harvest is half of the result. Case BBL-0504 below (a fit 24-year-old, BMI 23.2) is the closest published case to a slim build, and the case descriptions list the volumes placed, from 380 cc per buttock upward.
Risks and safety
A skinny BBL carries the risks of liposuction and of fat grafting together, and one of them is larger on a lean frame: with less fat under the skin to begin with, over-suctioning shows sooner.
What can go wrong
- Dents, ridges and shelves in the donor areas where too much fat was taken
- Bruising, numbness, a seroma, a hematoma or an infection at the entry points
- Firm lumps (fat necrosis) where fat does not survive, asymmetry, or more resorption than expected
- Fat embolism, fat entering the large veins of the buttock: the cause of the BBL deaths, traced to fat injected into or beneath the gluteal muscle
- A blood clot, bleeding, or a reaction to anesthesia: rare, and the reason you walk the first night
How Dr. Cat reduces these risks
- The harvest spread across many areas and stopped early in each, with thin cannulas, no heat devices and a thin cushion of fat left under the skin
- Fat placed only in the layer under the skin, above the muscle, with a blunt cannula, in slow passes
- Fat washed rather than spun and placed in fine threads rather than pools
- Careful selection: donor fat, a stable weight, a BMI of 30 or below, no nicotine, medical clearance and lab work
- A physician anesthesiologist in her QUAD A accredited surgery center, one patient that day, within the five-liter legal limit
- Walking the first night, the garment and foam, the six-week sitting rule with the pillow, no lymphatic massage, and follow-up visits
How much does a skinny BBL cost in Beverly Hills?
Dr. Cat's surgeon's fee for liposuction with fat transfer to the buttocks is $56,500 to $63,500, and a skinny BBL is quoted at that fee rather than below it: the fee follows the operating time and the number of areas liposuctioned, not the volume transferred, and a slim patient needs a full liposuction 360 harvest, often with the thighs. The operating room and the board-certified physician anesthesiologist are quoted at your consultation, once Dr. Cat has examined you and set the operating time your surgery needs: operating time varies with your anatomy and with the areas treated together, so it is set by her, not by a price list. Surgeon's fees are typical ranges: a more complex case can cost more, and your fee is set at your consultation and is final only in your written quote. The quote is itemized and includes your garment, your pillow and your follow-up visits. Lipo 360 alone is a $45,500 to $52,500 surgeon's fee, and fat transfer to the hips alone $35,000 to $45,000.
The price reflects one surgeon who performs every stroke herself, one patient per surgical day, a physician anesthesiologist and an accredited facility. Please do not fall for cheap or fast surgery: fat placed in the wrong layer can be fatal, and liposuction done fast on a lean frame leaves dents that are often irreversible. Our BBL cost guide explains what a quote is made of.
Consultation fee and financing
A consultation with Dr. Cat is $500, or $1,500 for a revision consultation, credited in full toward your surgery. The practice partners with Cherry, its primary financing partner, and also accepts CareCredit and Alphaeon Credit; see financing and payment plans. A skinny BBL is cosmetic and not covered by insurance.
Your consultation with Dr. Cat
It starts with a free 15-minute discovery call with our Patient Concierge. At the consultation, in person on Rodeo Drive or, for out-of-town patients, by private video consultation through OnPatient, the HIPAA-secure patient portal, Dr. Cat reviews your health and weight history, then examines the pinch of fat at each donor area, the shape of the buttock from the side and the back, whether the hips dip, and the quality of the skin. On a slim patient this examination decides the plan. She tells you where the fat will come from, roughly how much can be transferred, and what your frame can reach, and you leave with an itemized written quote. About a third of her patients travel from out of state or abroad.
