A Brazilian butt lift is liposuction plus fat transfer: the same fat removal, followed by moving the purified fat into the buttocks and hips. So the question is not which operation is better but whether you want fat removed only, or removed and moved. If you like your buttock shape and want a slimmer waist, back or thighs, liposuction alone is your operation; if you also want more projection or a rounder hip line, you need the transfer, and that is a BBL.

What Liposuction Does
Liposuction in Beverly Hills with Dr. Cat is tumescent liposuction: a numbing solution goes in first, then a thin cannula through an incision of a few millimeters removes the fat in even passes. She leaves a thin cushion of fat under the skin on purpose, because over-suctioned skin sticks to the muscle and dents; correcting that is a large part of her revision liposuction practice. She does not use laser, ultrasound or other device-assisted liposuction, having seen the burns and scar tissue they leave.
Liposuction changes the amount of fat and nothing else. It does not tighten skin or repair separated muscles; those are tummy tuck problems, covered in tummy tuck vs liposuction. What a BBL Does
A Brazilian butt lift in Beverly Hills starts with the same liposuction, most often a lipo 360, and the harvested fat is washed and injected into the buttocks and hips in many small passes. Dr. Cat describes it as working with clay: liposuction scoops out and thins the waist, and the transfer adds that volume back where you want it. The waist becomes the narrowest point and every area blends into the next; the change is a new waist-to-hip ratio.
The transferred fat is living tissue, so how it is harvested, handled and placed decides how much survives. In Dr. Cat's experience more than 80 to 90 percent of the transferred fat survives, which is why her patients rarely need a second round. Once it has a blood supply it stays, and it gains and loses with your weight. A BBL does not remove loose skin and cannot add more than the donor fat allows. Lipo 360 vs BBL
This is the comparison most people mean. Lipo 360 is the harvest: upper and lower abdomen, flanks and hips, and upper, mid and lower back, treated as one contour. A BBL is lipo 360 plus the transfer. After lipo 360 alone the waist is narrower, the back is flatter and the buttocks are unchanged, which makes them look more prominent from behind by contrast; for many patients that is the result they wanted. What lipo 360 alone cannot do is add projection, round out a flat or square shape, or fill hip dips, because those are volume problems and liposuction only takes volume away.
Patients who have booked a BBL sometimes ask whether to keep the lipo 360 and skip the transfer. It saves the sitting rule and hours of surgery, but the fat is discarded and cannot be banked, so decide before surgery.
How to Decide Between Liposuction and a BBL
The shape you want
Describe the outcome, not the procedure. A smaller version of the shape you have now is liposuction. A smaller waist with a fuller, rounder hip line, or projection you do not have today, needs the transfer. Dr. Cat asks patients to bring photos of buttocks they like and tells them when a photo has been edited; many goal photos on social media show results no surgery produces.
How much donor fat you have
Only the fat that survives counts, so the harvest has to be generous. Dr. Cat examines each donor area at the consultation and estimates what can be moved; she has turned away patients who did not have enough fat to remove. When the fat is not there, the answer is not a device: Dr. Cat does not place buttock implants, does not inject Sculptra or any filler into the buttocks, and does not perform buttock lifts. Fat is her only approach, because it is your own tissue, it becomes living tissue once it takes, and the liposuction that harvests it is half of the result; a lean patient is told what a subtle change can reach rather than offered an implant. The ceiling is legal as well as anatomical: liposuction in one surgery is capped at five liters.
Your BMI and the skinny BBL
Neither operation is weight loss, and both last longest at a stable weight, so anyone on a weight-loss plan should be near their goal first. BBL candidates should have a BMI of 30 or below. Slim patients are not excluded: a skinny BBL harvests from a lipo 360 and transfers a smaller volume for a subtle change.
How much recovery you can take
Liposuction asks little: walking within hours, work within days, a garment for six weeks. A BBL asks for six weeks without sitting directly on the buttocks, which means a pillow under the thighs, sleeping on your stomach or side, and a job that can absorb it. If that is not possible, choose liposuction alone or schedule the BBL for a season when it is.
When Dr. Cat Recommends Liposuction Alone, and When She Recommends a BBL
She recommends liposuction alone when the buttocks already have the shape the patient likes, when the request is one or two areas rather than a full contour, when donor fat is limited and a subtle change is not wanted, and when the six-week sitting rule cannot be kept. She recommends a BBL when the goal is a changed proportion, there is enough donor fat to reach it, and the patient can commit to the recovery. A patient whose goal is only a slimmer waist is not talked into a transfer. Two rules she does not bend: a BBL is never performed on the same day as a tummy tuck or a breast augmentation (those combinations are staged on separate days for safety), and she performs every surgery herself.
BBL Safety: Why Technique and the Surgeon Matter
The BBL is the higher-risk operation. The serious complication unique to gluteal fat grafting is fat embolism: fat entering a large vein in the buttock and travelling to the lungs or heart, which happens when fat is injected into or beneath the gluteal muscle, where the large vessels run. The rules that changed its safety record are technique rules: fat placed only in the layer under the skin, above the muscle; blunt-tip cannulas, not sharp needles; the cannula angled upward and parallel to the skin; slow passes as the cannula is withdrawn; ultrasound guidance where available. A board-certified plastic surgeon who follows them, in an accredited facility with an anesthesiologist in the room, carries a different risk profile from the unaccredited facilities that most of the complications in the news trace back to.
Red flags for either operation: a price far below the market, pressure to book, a provider who will not discuss risks, a facility that will not name the operating surgeon in writing. Dr. Cat sees the results in her revision practice; uneven BBL results are among the hardest to correct.
Results and Before-and-After Photos
The galleries show the difference better than any table. In the liposuction before and after photos, look at the rear view: the waist and back get smaller and the buttocks keep their shape. In the BBL before and after photos, the same view shows a narrower waist and fuller hips and buttocks. Compare like with like: liposuction refines over three to six months, BBL swelling is largely gone by week six, and the shape keeps settling for up to a year.
Recovery Side by Side
Liposuction of one or two areas is a same-day surgery: you go home with someone to stay with you, and only lipo 360 earns the first night at the aftercare facility. After liposuction you are walking within hours, most patients return to work within a few days, and a custom-measured compression garment is worn for six weeks to press out the fluid the cannula leaves behind. After a BBL the first night is spent at an aftercare facility with nursing. Most patients are back at desk work at about one week and doing most normal activities by four weeks. What is the difference between liposuction and a BBL?
Liposuction removes fat and discards it. A BBL removes fat with liposuction, purifies it and injects it into the buttocks and hips. Every BBL includes liposuction; liposuction does not include a transfer. One makes an area smaller; the other changes the proportion between the waist and the hips.
Is lipo better than a BBL?

What does a BBL look like after 10 years?
The surviving fat is permanent. Ten years on, the buttocks look as they did at one year, adjusted for what the body has done since: weight gain enlarges those fat cells, weight loss shrinks them, and skin ages as it does everywhere. Will lipo get rid of a hanging belly?
