Love handles are the pads of fat that sit over the hip bones on either side of the waist, and they stay because your genes decide where fat is stored last and lost last; no diet or exercise can choose the spot. Liposuction can, because it removes the fat cells from that exact place, and it is the only treatment that reliably does.
What Love Handles Are, and Why the Name
The name is old slang for the rolls at the waist a partner can hold on to; the medical term is flank fat. The pockets sit above and behind the hip bones, where the lower back meets the side of the waist, and they are subcutaneous fat, the layer under the skin that you can pinch. That distinction matters. Visceral fat sits deep inside the abdomen around the organs, carries most of the health risk of a large waist, and responds to diet, exercise and sleep; no surgery treats it. Subcutaneous fat is the layer liposuction removes, and it is also the layer that shrinks last.
Muffin Top: The Same Fat, Seen From the Front
A muffin top is what love handles and lower belly fat look like when a waistband presses on them: the roll spills over the top like the cap of a muffin. It is not a separate depot, and it shows on slim people, because a tight waistband will find whatever soft tissue sits above the hip. Treating the flanks and the lower abdomen as one continuous shape is what removes it; treating the front alone leaves the sides, and the roll simply moves.
Why Diet and Exercise Plateau Here
You cannot spot-reduce. When you lose weight, fat cells across the body release stored fat in response to hormones that reach every cell at once, so the fat you burn during a side plank comes from everywhere, not from the flank you are working. Where the loss shows first and last is set by genetics; for many women the flanks, the lower belly and the outer thighs hold on, and for men the flanks and the lower back. Fat cells in these depots carry more of the receptors that block fat release and fewer that trigger it, which is why the last ten pounds leave the face before they leave the waist. Exercise still lowers total fat, builds the muscle beneath, and makes you a safer surgical candidate. It just cannot pick the pocket.
The number of fat cells you carry is set by early adulthood; weight changes make the cells larger or smaller, not more or fewer. Liposuction removes cells, which is why the result lasts at a stable weight.
Love Handles When You Are Lean
Plenty of Dr. Cat's liposuction patients are fit: runners, dancers, bikini competitors. They come in because their body fat is low everywhere except one place. That is genetics, not a failing, and it is exactly the case liposuction was designed for: a defined pocket on a stable-weight body with good skin. A high or flared hip bone can also make a small pad look larger, because the fat sits on a shelf; the fat still comes out, and Dr. Cat shapes the transition into the hip rather than chasing the bone.
Love Handles in Men
Men store fat around the trunk, so the flanks, the lower back and the abdomen fill first and empty last, and the spare tire is a flank problem as much as a belly problem. Men also carry more visceral fat, which is why a man's waist can shrink with diet while the pinchable roll over the belt stays. Liposuction of the flanks and lower back, sometimes with the abdomen, is the usual plan. Back fat in men is dense and fibrous, so Dr. Cat uses power-assisted liposuction there, and the thick skin of a man's flank usually retracts well afterward.
What This Page Covers, and What It Does Not
Love handles, muffin top, the lower belly, bra and back rolls, inner and outer thighs: the stubborn pockets liposuction treats. Two other pockets have their own pages, because the answer is different: arm fat is treated with arm liposuction only when the skin still has good tone (Dr. Cat does not perform arm lifts, and says so when loose skin is the problem), and a double chin is treated with chin liposuction and sometimes a neck lift. If your belly problem is loose skin rather than fat, read about apron belly.
What Does Not Fix It
Creams, wraps and fat-burning supplements do nothing to fat cells, and waist trainers move tissue only while you wear them. CoolSculpting freezes fat cells in a pocket the applicator can grip; it can shrink a small, pinchable pocket by a fraction per session, usually needs several sessions, cannot shape the transition into the hip, and carries a rare risk of the treated pad enlarging and hardening (paradoxical adipose hyperplasia), which is then treated with liposuction. Kybella dissolves fat by injection but is approved only for under the chin. We offer neither; our non-surgical menu is skin tightening and injectables, and we would rather say so than sell you six sessions of something that will not do what you want.
Laser, ultrasound and "sculpting" liposuction (laser lipo, SmartLipo, Vaser, AirSculpt, ab etching) are surgery with a heat source added, and Dr. Cat does not use them, because she has seen the burns, the uneven results and the scar tissue left where abs are etched. She uses tumescent liposuction, with power-assisted cannulas for dense areas.
The Options
Liposuction
For one to three defined pockets on a stable-weight body with good skin. Through incisions a few millimeters long, Dr. Cat removes the fat with a thin cannula and leaves a small cushion under the skin so the surface stays smooth; over-suctioning causes most of the dented, wavy results she repairs in revision liposuction. The law limits removal to five liters in one session. Read about liposuction in Beverly Hills for the surgical detail; for a single small area, awake liposuction under local anesthesia is an option.
Lipo 360
When the problem is the whole waistline rather than one pocket, lipo 360 treats the upper and lower abdomen, the flanks, the hips and the upper, mid and lower back as one circuit, so the waist is the narrowest point and every area blends into the next without a shelf. It is the operation most of our muffin top patients need, and the fat it removes can be used: if you would rather move it than lose it, a Brazilian butt lift transfers it to the buttocks and hips. Our comparison of liposuction vs BBL covers that decision.
Tummy Tuck When the Skin Is Loose
Liposuction removes fat and leaves skin. If your lower belly has stretched skin or separated muscles from pregnancy, suctioning the fat out leaves the skin looser, and a tummy tuck is the right operation, sometimes with conservative liposuction of the flanks in the same surgery. Our comparison of liposuction vs tummy tuck explains how Dr. Cat decides; the short version is the pinch test. Thick and firm is fat. Thin and draping is skin.
What Dr. Cat Looks For at the Consultation
She pinches each area to measure how much fat is there and how the skin above it retracts, looks at how the pockets connect to the hips, asks about your weight history, and tells you plainly whether liposuction will give you the result you want, whether a different operation would, or whether neither would. She has turned patients away who did not have enough fat to remove: a few centimeters of fat in an area can be treated, less cannot. Candidates should be close to a stable goal weight. The consultation fee is $500, credited toward surgery; the discovery call before it is free.
Before and After
Look for a body like yours in our liposuction before and after photos: the back view is where love handles show, and the side view is where a muffin top does.
Recovery in Brief
Liposuction of one or two areas is a same-day surgery: you go home with someone to stay with you rather than to the aftercare facility, which is reserved for lipo 360 and larger operations. You are up and walking within hours and back to desk work within a few days. A compression garment worn for six weeks squeezes flat the tunnels the cannula leaves so the skin settles smoothly, and Dr. Cat advises against painful lymphatic massage. Swelling hides the result at first; the contours refine over three to six months.
Cost in One Line
Dr. Cat's surgeon's fee is $10,000 for a single area on its own, or about $6,500 to $7,500 per area when two or more areas are treated, with anesthesia and the facility added on your quote; lipo 360 is about $44,000 to $51,000 all in. Every figure is quoted at your consultation, and our liposuction cost guide explains what moves the price.
Frequently Asked Questions About Love Handles and Stubborn Fat
Why do they call love handles?
It is old slang for the rolls at the sides of the waist that a partner can hold on to. The medical term is flank fat, subcutaneous fat over the hip bones and lower back, and the name says nothing about health or attractiveness.
How do I get rid of love handles?
Lower your total body fat with diet and exercise first. If the flanks are the last place to change and your weight is stable, liposuction removes the fat cells from that spot for good. No exercise targets the flanks.
Why do I have love handles but I'm skinny?
Genetics decide where fat is stored last and lost last. A lean body can hold a defined pocket at the flanks while everything else is at low body fat, and a flared hip bone can make a small pad look larger. That is the anatomy liposuction treats best.
Are love handles attractive?
That is personal, and plenty of people like them. This page is for people who want theirs gone and found that diet and exercise did not move them. Nobody needs surgery for a normal fat pad; the decision is yours alone.
Is liposuction a weight-loss procedure?
No. Liposuction reshapes specific areas by removing stubborn fat; it is not a treatment for obesity or a substitute for diet and exercise, and the law limits one session to five liters. The best candidates are already close to a stable goal weight with good skin elasticity.
Will liposuction tighten loose skin?
No. Liposuction removes fat; it does not tighten skin. If loose skin or separated muscle is the main issue, a tummy tuck, not liposuction, is the right procedure.
Does fat come back after liposuction?
The removed fat cells do not grow back. If you gain weight, the remaining cells everywhere enlarge, so the treated area gains proportionately less. A large gain can still change the result, which is why a stable weight protects it.
